Our Pet Tales (entry deadline extended to January 15, 2025)

Our Pet Tales application can be completed numerous ways:
1) Complete the fillable PDF application and save to your desktop.  Then, email the application and pet photograph as directed.
2) Print the fillable PDF application.  Then mail the completed application and pet photograph as directed.
3) The application may also be picked up at either the NHS Thrift Shop or Shelter.
